4.2.1.2 二位相折返的二种机制:【9,10,15-20】
心脏性猝死的2相折返的机制和类型,可按离子流机制,心电图和临床特点区分为QT间期病理性延长和QT间期正常或病理性缩短二类:
一类是以长QT综合征为代表的,Ikr、Iks等外向钾电流外流减少或钙超载,后除极触发机制诱发的2位相折返,心电图表现为QT间期延长、QT长短交替、T波交替、Tp—Te延长和QT离散度增大等,临床常见于心室肥厚、陈旧性心梗、慢性心衰和各种药物所致的QT间期延长等。
另一类以J综合征为代表,心外膜瞬时外向钾电流(Ito)增加,穹窿消失与心内膜的电位差增大,诱发2位相折返性室速、室颤。心电图特点是J点抬高、J波形成,ST段抬高,且与T波的上升支融合为一体弓背向下,但QT间期正常或缩短,可同时有T波交替、Tp—Te延长和QT离散度增大等。临床常见于急性冠脉综合征超急期的J波形成和Brugada综合征、ERS和SQTS。
4.2.2 预后和预防4.2.2.1 预后 心电图Tp-e是可以间接反映TDR变化的临床指标【10-31】
心脏性猝死是危害人类的最大的死亡原因,占心血管病死亡总数的64%。心脏性猝死的心电图的高危预警指标有:QT间期延长(≥460/480ms), QT间期缩短(≤350/330ms), QT离散度增大(≥80/100ms),T波交替(TWA),和心率变异、心室晚电位等。
近年来特别重视T波顶点(T-peak)到T波终点(T-end)间期(Tp-e)增大(≥80/120ms),和J波异常。因为Tp-e是目前唯一的心电图可反映心室复极跨壁离散(TDR)的指标。TDR增大是公认的最能预警的指标,但是TDR只有在冠脉灌注的心肌组织块的细胞电生理研究时才能获得,因此心电图Tp-e间接反映TDR具有十分重要的临床意义。T波形成的离子机制决定于复极期的外向IK1、IKr、IKs和内向INa、ICa等离子流。心电图T波形成机制,概括一句话就是心外膜、中层和心内膜三层细胞复极二位相-三位相电位的动态代数和。心内膜(或/和中层)与心外膜的电位差为正,T波直立;为负T波倒置。心外膜复极先结束,相当于T波的顶点;心内膜梢后结束,M细胞结束最晚,相当于T波的终点,因此T波的顶点至终点的间期(Tp-e)可以代表复极离散度(TDR) 【12-13,19-29】 。
4.2.2.2 预防 一级预防为主-辅以ICD防治VT/VF和SCD
流行病学资料显示:心血管病死亡的64% 是SCD,其中的60%是急性缺血所致。因此积极预防冠心病和动脉硬化,以调理生活方式和药物等一级极预为主;对有适应症的病人积极开展ICD防治二位相折返性VT/VF和SCD【31-45】。
